For
treatment
According
to applicable law, "treatment" means the provision,
coordination and management of healthcare services, including
consultation between healthcare providers and others relating to persons
whom we may examine or become engaged in consultation. Since this
definition appears to describe our services, we wish to inform potential
consumers that NANpc does not provide psychological treatment or
intervention in the traditional sense except under unusual
circumstances. NANpc offers neuropsychological and psychological
examination and consultation services for clinical-medical, legal and
administrative purposes. Psychotherapy, counseling, cognitive
rehabilitation and other psychological and neurobehavioral treatment
modalities and interventions are not provided. There are no
"on-call" doctors for emergency or urgent consultations. No
"doctor-patient" treatment relationship is implied or offered.
Our primary purpose is to provide neuropsychological examinations and
consultations to examinees and referring parties. NANpc does not assume
the role of a primary care or treating doctor with any of our examinees.
We also do not prescribe or manage medications.
We
may disclose your PHI to others who provide treatment to you. We
are likely to share your information with your referring doctor,
attorney or other referral source.
If
you are being treated by a team we can share some of your PHI with them
so that the services you receive will be coordinated. We may refer you
to other professionals or consultants for specialized services that we
cannot offer. When we do this, we need to tell them some things about
you and your conditions. We will obtain the records of some of your
treating healthcare providers and others and that information will
become part of your record.
For
payment
We
may use your information to bill you or others in order to be paid for
the services we provide. Please be advised that advance payment is
required for all services unless alternate arrangements are made prior
to scheduling your appointment(s) and guaranteeing the dates of service.
Third party payers are usually given copies of examination reports
including history and diagnostic information. They may need dates of
service, service location information and so forth. NANpc will only
assist you in billing insurance companies. Any questions that you may
have about insurance coverage, deductibles, and covered services should
be addressed by you with your insurer. NANpc is not a Medicare provider
and special forms must be read and signed by Medicare
beneficiaries who desire us to provide services outside of
Medicare reimbursement.
For
health care operations
We
must use your PHI to compile statistics about our practice, to monitor
our performance, or for research purposes. We may be required to supply
some information to government health agencies or others so they can
study disorders and make plans for service provisions. If we do, your
name and identity will be removed from what we send.

Other uses in health care
We
may use and disclose medical information to schedule and reschedule or
remind you of appointments for examinations and/or consultation
services. For IME's and some other types of legal or administrative
examinations, we may be required to provide information to allow another
party to schedule and reschedule your appointment(s). We may use and
disclose your PHI to tell you about or recommend certain treatments or
alternatives that may of interest to you. We may also use and disclose
your PHI to tell you about health-related benefits or services that may
be of interest to you.
NANpc
is involved in clinical research activities from time to time. We may
use or share your information to do research. In all cases, your name,
address, and other information that reveals who you are will be removed
from the research information and will not be available to third party
research consultants.
